Terms of service.

 

1. Scope of Services

Plato Unida Nutrition™ provides trauma‑informed, culturally honoring nutrition therapy and nutrition counseling. Services may include assessment, education, goal‑setting, and collaborative care planning. I do not provide medical diagnoses, emergency services, or treatment outside the scope of nutrition therapy.

2. Client Responsibilities

Clients agree to:

  • provide accurate and complete health information

  • complete required intake forms before sessions

  • obtain a referral from a licensed provider when required

  • communicate respectfully and in good faith

  • attend scheduled appointments on time

3. Payment & Tiered Pricing

Payment is due at the time of booking unless otherwise stated. Plato Unida Nutrition offers a tiered pricing model (Supporter, Sustainer, Equity, Solidarity). Clients are trusted to choose the tier that reflects their current financial reality.

Packages are non‑refundable and must be used within 6 months of purchase. Unused sessions do not roll over or convert to other services.

4. Cancellation & Rescheduling Policy

Your appointment time is reserved just for you. If you need to reschedule, please provide at least 24 hours’ notice.

  • Cancellations with less than 24 hours’ notice

  • Missed appointments

  • Repeated last‑minute rescheduling

…may result in the session being counted as used, forfeited if part of a package, or subject to a no-show fee.

5. Referral & Documentation Requirements

Initial appointments require final approval. Before your session can be confirmed, I may need to:

  • review your intake forms

  • verify insurance information (if applicable)

  • obtain a signed referral from your provider

Referral timelines vary and may delay scheduling. Plato Unida Nutrition is not responsible for delays caused by provider response times.

6. Between‑Session Support

Packages include light resource sharing only between sessions. Direct coaching, check‑ins, crisis support, or ongoing messaging are not included.

7. Telehealth Services

Telehealth sessions require a stable internet connection and a private space. By participating in telehealth, you acknowledge the risks of technology interruptions, delays, or unauthorized access despite reasonable safeguards.

8. Not Emergency or Crisis Care

Plato Unida Nutrition does not provide emergency services. If you are experiencing a medical or mental health emergency, call 911 or go to your nearest emergency room.

9. Privacy & Confidentiality

Your information is protected under HIPAA. A full Notice of Privacy Practices is available upon request. Electronic communication (email, telehealth platforms) carries inherent risks despite reasonable security measures.

10. Communication Policies

Email is intended for scheduling and administrative questions only. Clinical questions, coaching, or therapeutic support must occur during scheduled sessions.

11. Termination of Services

Plato Unida Nutrition reserves the right to pause or end services if:

  • referrals or documentation are incomplete

  • the client’s needs fall outside the scope of practice

  • there is repeated non‑attendance or boundary violations

  • a higher level of care is clinically indicated

Clients may end services at any time, understanding that packages are non‑refundable.

12. Agreement to Terms

By scheduling, purchasing services, or participating in care, you acknowledge that you have read, understood, and agree to these Terms of Service.
